We currently direct bill to several major insurance companies. Please note that every insurance plan varies and some do not allow us to electronically submit and/or receive payment on your behalf. In those cases, we will help guide you through the submission process.
Information we will need in order to submit for you:
· Insurance card or info (policy/contract number, Certificate/ID)
· Doctor’s referral if required. (It is best to check with your insurance company if you are required to have one).
· Policy holder’s name and date of birth if not self (spouses and children)
Note: that if both you and your spouse/partner have coverage: submission to your insurance has to take place first. For children: if both parents have extended health benefits, the parent whose birthday comes first in the year will be the one that is billed first. Please note that we are unable to do co-ordination of benefits.
